Position Overview:
- We are seeking a dedicated Clinical Outreach Liaison to join our Partial Hospitalization Program (PHP) and Intensive Outpatient Program (IOP) for Substance Use Disorder (SUD) treatment.
- The ideal candidate will be responsible for building and maintaining relationships with referral sources, promoting our services, and facilitating smooth patient transitions into our program.
Key Responsibilities:
Develop and maintain relationships with healthcare providers, hospitals, and community organizations to generate referrals
- Educate referral sources about our PHP/IOP SUD services and treatment approach
- Conduct initial assessments of potential clients to determine program suitability
- Coordinate with clinical team to ensure seamless admission process for new patients
- Participate in community events and professional conferences to increase program visibility
- Maintain accurate records of outreach activities and referral outcomes
- Collaborate with internal teams to improve outreach strategies and program offerings
- Minimum 1 year experience in substance use disorder treatment or related healthcare field
- Strong understanding of SUD treatment modalities and continuum of care
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Valid driver's license and reliable transportation
