Summary :
The Care Navigator is responsible for completing resource referral and follow-up calls to clients based within northern Virginia. Care Navigation provides a seamless solution that helps individuals and families access care in their communities, ensuring individuals and their families never have to navigate their needs alone. The Care Navigator bridges the Regional Crisis Call Center back to the available resources within the public mental health system and human service access points.
Primary Duties :
- Complete scheduled follow-up call contacts with individuals to facilitate care transitions.
- Provide resource referrals to individuals seeking them and utilize warm transfers when appropriate.
- Build and maintain resource and referral library for use by care navigators, follow-up crisis workers, and the follow-up supervisor.
- Document all follow-up contacts appropriately and thoroughly using the correct documentation portals or platforms.
- Participate in opportunities for professional growth and development to stay current on job-specific knowledge, particularly the most current resources and referrals available for clients.
- Participate in regional meetings as requested.
- Respond to crisis contacts by greeting clients and establishing rapport by being accepting, empathetic, objective, non-judgmental, non-directive, and responsive to the feelings of the callers or texter.
- Explore the individual's emotional and mental state using active, positive, comforting, and problem-solving language; following established protocols; identifying warning signs; and assessing the degree of clients' risk.
- Provide safety and security for individuals by assuring them of their confidentiality and privacy and ensuring that any information shared by the client is kept confidential.
- Accurately reflect crisis contact interactions in corresponding contact reports throughout and at the end of each contact.
- Complete administrative tasks as assigned by the CrisisLink Follow-Up Supervisor or Lead Clinician.
