HopeLink is seeking a dedicated and experienced Program Manager to oversee our Community Housing program. In this role, you will play a vital role in supporting individuals with serious mental illness achieve stability and independence in their homes.

About the Role :
Under the direction of the Director of Community Support Services, the Program Manager for Community Housing administers and supervises residential programming per Licensure, CARF and grant requirements. In addition, the position provides counseling, community living skills training, crisis intervention and case management to individuals with serious mental illness in multiple residential settings. Coordinates rehabilitation activities in community settings.
Responsibilities :
- Provide individualized support, counseling, and case management to residents in permanent supported and independent housing settings.
- Develop and implement engaging programs that help residents reach their rehabilitation goals.
- Supervise and mentor 2-3 Community Support Specialists.
- Directly teach community living skills to residents.
- Ensure residents' safety and well-being through monitoring and intervention.
- Collaborate with other service providers to advocate for residents' needs.
- Assist with securing resources necessary for residents' stability.
- Conduct regular reviews and ensure clinical documentation meets all standards.
- Represent the agency at public forums and facilitate residents' integration into the community.
