Description
The Assisted Transportation Driver is responsible for safely transporting clients (particularly the elderly and frail) to medical and/or other appointments as assigned. This position ensures vehicles are in safe operating condition, maintains client and transportation records and adheres to the HOPES CAP, Inc. Policies & Procedures.
ESSENTIAL FUNCTIONS:
- Transport clients to and from home or other residence to and from designated appointment and aide clients in safely getting in and out of the vehicle as well as in and out of residence/facility.
- Operate vehicle in safe manner and in compliance with New Jersey laws and regulations.
- Maintain appointments and adhere to agreed upon transportation schedule and communicate any schedule changes to assigned staff.
- Make decisions regarding safe operation and precautionary measures of vehicle in adverse weather conditions.
- Clean and fuel vehicle as needed and according to schedule and inform Lead Driver when vehicle is in need of maintenance and repairs and maintain appropriate temperature and cleanliness within the vehicle.
- Apply preventative maintenance on vehicles (i.e., check oil, fluids, air pressure in tires, etc.) as assigned and send vehicle to required inspections; complete appropriate paperwork as assigned.
- Conduct client intake – which includes assessment of clients’ needs and plans for continuing services.
- Maintain up to date case management records on all transportation clients.
- Keep accurate records of mileage and submit mileage report to Community Services Manager as assigned.
- Submit all other relevant documentation and data regarding Community Services Transportation as assigned.
- Submit all original invoices, estimates, and receipts to assigned staff.
- Receive phone calls, take accurate messages, and route calls to appropriate personnel.
- Attend meetings and trainings for the purpose of receiving information on new and/or improved procedures.
- Maintain confidentiality in regard to all agency and program records.
- Conduct and document follow-up contacts for all services provided, identify, resolve and communicate obstacles and challenges to supervisors promptly.
- Actively participate in special community events and agency-wide initiatives including development events and activities.
- Perform any other duties as assigned in support of agency and program needs and operations.
Qualifications
MINIMUM REQUIREMENTS: (Must be fully met in order to qualify for position)
- High School Diploma or equivalent combination of education and experience with a valid New Jersey State Driver’s License.
- 3 yrs.’ experience driving a multi passenger vehicle and experience in vehicle maintenance and repair.
- Demonstrate knowledge and understanding of applicable regulations and standards as it applies to New Jersey Commission of Motor vehicles.
- Ability to speak, read, write, comprehend, process, interpret follow and carry out instructions spoken in the English language and written in a variety of documents and reference relevant to the job.
