What are the responsibilities and job description for the Sleep Center Intake Specialist position at HopeSource?
Responsibilities
- Completes registration forms
- Enter all data for individuals into Coordinated Entry
- Assign individuals to units and record which unit is assigned for the night
- Communicate camp rules and seek understanding of the rules
- Monitor camp activities from 6 : 00pm to 10 : 00pm or 6 : 00am to10 : 00am and coordinate with overnight security who will monitor the camp from 10 : 00 am to 6 : 00 am
- Leave any relevant information in the logbook for the next shift
- Engage in regular client outreach efforts in accordance with the outreach calendar.
- Perform targeted outreach for specific individuals and / or locations as required.
- Weekend work required
