Located in the heart of Chillicothe, Hopewell Grove Rehabilitation and Healthcare Center is a premier skilled nursing facility dedicated to providing high-quality, compassionate care to our residents. We are currently seeking a skilled and compassionate MDS Coordinator to join our team. This position is integral to ensuring the accurate and timely assessment of residents’ needs, guiding care plans, and supporting quality care across the facility.

Position Overview : As the MDS Coordinator at Hopewell Grove Rehabilitation and Healthcare Center , you will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments, ensuring compliance with federal and state regulations. You will work collaboratively with the interdisciplinary team to develop personalized care plans based on comprehensive resident assessments, aiming to enhance the quality of care and life for each resident. Your role will be pivotal in ensuring the facility's compliance with healthcare regulations, improving quality of care, and maintaining optimal outcomes for our residents.

Key Responsibilities :

  • Complete accurate and timely MDS assessments for all residents in accordance with federal and state regulations.
  • Develop and review individualized care plans based on MDS assessments and collaborate with the healthcare team to ensure all residents’ needs are met.
  • Ensure accurate coding of MDS data and submission to the appropriate authorities.
  • Monitor and assess resident outcomes to ensure that care plans are effective and aligned with residents’ evolving needs.
  • Collaborate with nursing staff, physicians, social workers, and other healthcare professionals to create and update care plans.
  • Review and analyze resident assessments to identify potential care issues and initiate appropriate action.
  • Assist in preparing for state and federal surveys, ensuring that all documentation and care protocols are in compliance.
  • Train and mentor staff on the MDS process and ensure adherence to established guidelines.
  • Provide education and support to families regarding the MDS process and the role it plays in resident care.
  • Participate in quality assurance initiatives and contribute to performance improvement activities aimed at enhancing care standards.
  • Ensure all MDS documentation is complete, accurate, and compliant with regulations.
  • Stay current on changes to MDS regulations and ensure the facility’s processes reflect industry best practices.
  • Qualifications :

  • Registered Nurse (RN) or Licensed Practical Nurse (LPN) with a valid license to practice in the state of Ohio.
  • Minimum of 2 years of clinical experience, with at least 1 year of experience in MDS coordination or a related role in a skilled nursing or long-term care setting.
  • Comprehensive knowledge of MDS assessment, care planning, and regulatory requirements.
  • Strong organizational and time-management skills, with the ability to handle multiple tasks efficiently.
  • Excellent communication skills, both written and verbal, with the ability to work effectively with residents, families, staff, and regulatory agencies.
  • Detail-oriented with strong documentation and analytical skills.
  • Ability to work independently and as part of an interdisciplinary team.
  • Strong understanding of quality improvement initiatives and the role of MDS in enhancing care outcomes.
  • Compassionate, resident-centered approach to care.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
  • Flexibility to work weekends, holidays, and on-call shifts as needed.
