Director of PACE Clinical Services
The Director of PACE Clinical Services is responsible for oversight and expertise of PACE clinical operations and systems, demonstrating leadership skills, identifying best clinical practices and standards while maintaining compliance with state, federal, and CDPHE regulations. This position focuses on ensuring that clear and effective systems, policies, and procedures are created and maintained across the PACE program that supports clinical staff in their work and serves as an integral member of the leadership team. This position is responsible for ensuring the integrity of clinical practice is upheld, coordinating education of new or revised clinical policies, procedures, and practices to ensure that PACE clinical staff are delivering care to participants that is in alignment with HopeWest policies and best practice. This role also serves as the Director of Nursing for PACE services and Home Health Administrator for the PACE home care program.
What is PACE?
PACE or Program of All-inclusive Care for the Elderly is a federal and state funded program that provides comprehensive, coordinated, and proactive whole-person care to help maintain and improve seniors' health so they can live safely in their homes. There are 180 PACE programs, serving more than 80,700 participants in 33 states.
The number of people aged 55 is increasing in Grand Junction, CO and the demand for eldercare professionals is on the rise.
What You Bring:
- Registered Nurse with current Colorado licensure; graduate from an accredited school of nursing. Bachelor’s Degree is required.
- At least 3 years of recent clinical experience in a PACE, community healthcare setting, or hospital is preferred.
- Valid Colorado driver’s license, unlimited access to reliable transportation, and adequate automobile insurance maintained throughout employment.
- Management and leadership experience in a health care setting.
- Maintains Home Health Administrator education requirements for PACE home health license.
- Current CPR certification is maintained throughout employment.
- Working knowledge of the Microsoft Office package including Word, Excel, PowerPoint, and Outlook.
