Dementia Support Nurse Practitioner
Part-Time or Full-Time Position
The Dementia Support or GUIDE (Guiding an Improved Dementia Experience) Model focuses on comprehensive, coordinated dementia care and aims to improve quality of life for people with dementia, reduce strain on their unpaid caregivers, and enable people with dementia to remain in their homes and communities. It will achieve these goals through Medicare payments for a comprehensive package of care coordination and care management, caregiver education and support, and respite services.
Dementia affected more than 6.7 million Americas in 2023, with 14 million projected cases by 2060.
The Dementia Support Nurse Practitioner (NP) is an advanced practice nurse, providing clinical oversight and medical care in accordance with state and federal practice laws.
This NP collaborates internally to establish realistic goals and a thorough, up to date beneficiary plan of care (POC). The NP serves as the primary dementia care provider for assigned panel of beneficiaries, providing dementia specialized health care and case management, focusing on disease management, while supporting wellness and management of minor, chronic and acute illnesses. The NP collaborates externally with attending medical providers, specialists, and other providers as applicable for the purpose of coordination of care to effectively meet the POC of beneficiaries.
What You Bring:
- Board certified Advanced Practice Nurse licensed in the state of Colorado, preferably Certified Family Nurse Practitioner or Geriatric Nurse Practitioner. Must have both APN and RXN licenses in Colorado.
- Must have dementia proficiency per CMS requirements as follows:
- Having at least 25% of their patient panel at some time in the past 5 years comprised of adults with any cognitive impairment, including dementia, or
- Having at least 25% of patient panel at some time in the past 5 years be comprised of adults aged 65 or older, or
- Having a specialty designation of neurology, psychiatry, geriatrics, geriatric psychiatry, behavioral neurology, or geriatric neurology.
- Controlled substance license with current DEA license number.
- Eligible or currently possess a Medicare/Medicaid UPIN number.
- Valid Colorado driver’s license, unlimited access to reliable transportation, and adequate automobile insurance is maintained throughout employment.
- Demonstrated exceptional interpersonal relations and problem solving.
- Recent clinical experience with adult and geriatric populations. Strong interest and experience in dementia and Alzheimer’s.
- Current CPR certification is maintained throughout employment.
- Effectively and respectfully communicate both verbally and in writing in a multitude of situations and settings, including highly emotional interactions.
- Effectively work with and build relationships with others.
- Adequate computer skills, experience in EMRs, and Microsoft office products.
- Effective communicator and team member at any meetings necessary.

- Competitive pay structure starting at $58-$60.00 per hour, depending on experience and education.
