This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare team at Horizon PACE will approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. Horizon has over 25 years’ experience providing care in Kentucky and is the largest provider of adult day health care services in the state. Horizon is opening 3 PACE locations in Kentucky, and we are excited to expand services to older Kentuckians. Under the supervision of the Adult Day Health Care Director, the Recreational Coordinator assesses the needs, interests and capabilities of participants and develops individualized therapeutic recreation plans. In addition, plans, organizes, directs and participates in a comprehensive therapeutic recreation program tailored to the general and individualized needs and limitations of frail elderly program participants. Provides leadership and support to Activities Coordinator.
Position Responsibilities:
- Conducts an in-person initial comprehensive recreational assessment. Coordinates with the Interdisciplinary Team to develop a comprehensive plan of care for each participant.
- Conducts an in-person reassessment semi-annually and as needed.
- Functions as a member of the Interdisciplinary Team. Maintains regular attendance at, and participants in, Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
- Plans and implements group and individual activities to improve or maintain the participants’ cognitive, physical, social, psychosocial, and artistic needs.
- Plans and implements activities for dementia-specific population. Oversees daily flow of dementia activities area. Activities may focus on small groups, one-on-one programming and sensory stimulation.
- Organizes community outings and special events at the Adult Day Health Center (ADHC). Duties include: arranging for participant transportation, entertainment, catering and involving other staff.
- Develops a monthly calendar of activities. Maintains record of monthly calendars as required for review.
- Evaluates program plans to promote effective programs according to client abilities and preferences within the ADHC daily flow of events.
- Provides recreational educational and training to support Activity Assistants, and other PACE Center staff members, as appropriate.
- Provides on-going support and training to all staff involved in dementia-specific programming related to the dementia disease process.
- Maintains participants’ medical record and fulfills Horizon PACE charting and reporting requirements
- Provides leisure education to participants and caregivers for activity modification, use of adaptive equipment, activity skill development, and recommended supplies for in-home use.
- Acts within the scope of his or her authority to practice
- Maintains general order in the activities area, including daily clean-up, routine supply inventory ordering, petty cash and overall general budgeting.
- Works cooperatively with other staff and departments to meet Horizon PACE participants’ needs.
- Assists participants with non-skilled Activities of Daily Living (ADL) as needed.
- Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
- Participates in and supports Quality Improvement Initiatives
- Performs related duties as required.

- Assist supervisor with various tasks to support department operations as directed; may also be assigned special projects
- Perform tasks which are supportive in nature to essential functions of the job, but which may be altered or re-designed depending on individual circumstances
- Assists participants and caregivers in filing grievances
Experience & Education:
- Bachelor's Degree in Recreation Therapy or related field preferred. Education or training in geriatrics, including dementia, preferred. Be legally authorized, currently licensed, registered or certified if applicable to practice in the State of Kentucky.
- Preferred to have two (2) years experience in a social or recreational program providing and coordinating services for a frail or elderly population.
