This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare team at Horizon PACE will approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. Horizon has over 25 years’ experience providing care in Kentucky and is the largest provider of adult day health care services in the state. Horizon is opening 3 PACE locations in Kentucky, and we are excited to expand services to older Kentuckians. Under the supervision of the Adult Day Health Care Director, the Licensed Practical Nurse or Registered Nurse provides medical and nursing care, ensuring high quality standards. Assists with coordination of services with members of the PACE service delivery team as needed. Triages and assesses participants as needed. Provides skilled nursing care to participants, implements orders, develops appropriate plans of care, provides participant, caregiver, family and staff teaching and communicates participant’s condition to caregivers, primary care and the interdisciplinary team
Position Responsibilities:
- Engages in team building. Coaches and mentor’s staff. Instructs and teaches staff as needed.
- Evaluates the overall effectiveness of adult day health care skilled services, implementing change and quality improvement as needed.
- Assists with the management of the flow of ADHC skilled services, ensures timely and appropriate care of participants.
- May assist with effectively communicating with Interdisciplinary Team community agencies at meetings and throughout the day.
- Assists the ADHC Director with signing off orders, ordering medications from pharmacy and disseminating orders and instructions to appropriate personnel. Ensures implementation of physician orders.
- Assists with Coordination of participant care with outside contracted service providers – nursing homes, assisted living, lab, oxygen, etc.
- Assists with communicates with weekend and after hour staff, following up appropriately.
- Manages and reviews participant’s records, charts, pertinent information and staff’s documentation. Documents as needed in medical records according to Horizon PACE standards.
- Performs general skilled nursing care to participants. Performs or supervises the administration of prescribed medications and treatments in accordance with nursing standards. Assist in treatment, examination and testing of participants.
- Responds to and perform Triage of incoming telephone calls. Oversees scheduling of outside appointments for participants when necessary.
- Oversees scheduling specialist appointments and assisting in hospital admissions and calling report to unit.
- Educates participants, family and caregivers in regard to care, medications and treatments.
- Educates staff regarding care and treatment for the elderly as needed.
- Acts within the scope of his or her authority to practice
- Assists with monitoring of clinic department expenditures and meets budgetary expectations.
- Follows all Horizon PACE program policies and procedures and applicable Occupational Safety and Health Administration (OSHA) safety guidelines.
- Protects privacy and maintains confidentiality of all company procedures, results and information about employees, participants and families.
- Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.
- Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
- Participates in and supports Quality Improvement initiatives.
- Performs other duties as required or requested.

Experience & Education:
- Graduate of a school of professional nursing program required. Current Kentucky Licensed Practical Nurse License or Registered Nurse License and Cardiopulmonary Resuscitation (CPR) certification required.
- Required to have a minimum of one year’s experience in working with the frail or elderly population required.

Pay: $28.00 - $30.00 per hour
