What are the responsibilities and job description for the Patient Care Coordinator position at Horizon Adult Health Care?
Horizon has over 25 years’ experience providing care in Kentucky and is the largest provider of adult day health care services in the state. Horizon has 3 PACE locations in Kentucky, and we are excited to expand services to older Kentuckians. The Patient Care Coordinator (PCC) shall coordinate adult health care services with the center staff, other health care providers, and the public. The PCC shall also conduct various forms of community education concerning ADHC and related services. They will also seek out and follow up on referral sources. They will also have knowledge of and be compliant with all company policies and regulations concerning service delivery and promote servant leadership.
Position Responsibilities:
- Educate the Community regarding ADHC and related services through relationships with other health care providers/agencies and other referral sources.
- Facilitate and maintain a strong, positive relationship with other health care agencies/providers by meeting with them weekly to assess potential clients and give input into current client status. This may be done through phone conversation, mail (status reports), or direct face-to-face meetings. One face-to-face meeting per month at a minimum.
- Facilitate and maintain a strong referral base by participating in marketing adult health care services to the public.
- Distribute marketing/education material.
- Follow up on Client Referrals from Various Sources
- Perform Intake Evaluation of Clients Physical and Psychosocial well-being.
- Develop a rapport with potential clients and client family/caregiver to ensure appropriate understanding of the facility and programs.
- Analyze medical and financial Home & Community Based Waiver criteria is met prior to new client admissions.
- Assist client, caregiver, or Case Managers with Medicaid applications upon request.
- Admit clients to the center as needed.
- Complete all required documentation and confer with any internal or external entities as needed.
- Assist with the Nurse Director to develop the most appropriate plan of care for the client.
- Maintain HIPPA/Privacy practices.
- Assist and maintain appropriate billing information to be coordinated with the central office.
- Provide written reports regarding intake, unit status, and operations of the Centers.
- Provide Personal Care
- Promote client independence by providing assistance, teaching, and prompting with personal care.
- Assist with feeding, as necessary.
- Assist with activities of daily living and incidental activities of daily living.
- Do or help clients with laundry, shopping, bill payment, and other IADL’s as needed.
- Obtain Vital Signs and report vital signs per Physician’s Order, Plan of Care, or as directed.
- Prepare Client Meals/Snacks
- Prepare center meals and snacks as directed by posted menus. Serve and perform clean up related duties.
- Perform meal service and maintain documentation and sanitation according to current CACFP regulations, timeline, and other applicable regulations.
- Assist with Center Maintenance
- Recognize, report, and correct safety issues.
- Assist and participate in center related activities and recreation to maximize client involvement.
- Actively engage clients in center activities and recreation.
- Acts only within the scope of his or her authority to practice.
- Follows Horizon Policies and Procedures and applicable OSHA safety guidelines.

- Perform tasks which are supportive in nature of essential functions of the job, but which may be altered or re-designed depending on individual circumstances.
Experience & Education:
- Preferred to have a high school diploma or equivalent.
- Preferred to have 1-year relevant experience.
- High level of organization skills
- A minimum of one year of experience in working with the frail or elderly population preferred.
