Horizon PACE is excited to announce an amazing opportunity to be part of Horizon PACE’s program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare team at Horizon PACE will approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. Horizon has over 25 years’ experience providing care in Kentucky and is the largest provider of adult day health care services in the state. Horizon is opening 3 PACE locations in Kentucky, and we are excited to expand services to older Kentuckians. Under the supervision of the Regional Director, the Center Director oversees and manages the daily operations of the Horizon Adult Day program including operations of the day center, social work, rehabilitation, recreation, and dietary. This includes ensuring that appropriate personnel are trained and perform their functions within the organization. Duties include assuring that operational services standards and budgetary criteria are met. As well as ensures operations are in compliance with all governing regulations.

Position Responsibilities:

  • Implementing and coordinating the program.
  • Continuously evaluating the participants' changing needs and making necessary program adjustments.
  • Supervising employees.
  • Providing appropriate in-service education.
  • Ensures that the job responsibilities, authorities, and accountability of all direct reports are defined and understood.
  • In the event that the dietitian is not employed full-time will be responsible for the operations of food service.
  • In conjunction with the Executive Director is responsible for maintaining compliance with all state and federal program requirements, obtaining and maintain certification under Centers for Medicare and Medicaid Services (CMS) as well as any other state or federal law or rules.
  • Assists in the development and implementation of the Horizon PACE LLC’s Strategic Plan.
  • Assists in program development and preparation of Horizon PACE LLC’s short-range and long-term plans, budgets and related information systems based on broad organization goals and growth objectives.
  • Is a member of the Horizon PACE LLC’s program Interdisciplinary Team (IDT) and acts as facilitator of IDT plan of care meetings.
  • Coordinates the Interdisciplinary Team’s care management process to ensure best decisions are made regarding the participant’s changing condition.
  • Ensures the coordination with home health, rehabilitative, long-term care and medical/specialty services according to participant care plans.
  • Assists Executive Director in development of annual operational and capital budgets for departments of accountability. Ensures financial benchmarks are met.
  • Assists Human Resources Manager in development and implementation of Horizon PACE, LLC’s program operational policies, procedures, protocols and regulations for direct participant care and support services.
  • Monitors operations to ensure consistent compliance with established quality assurance standards.
  • Assists Executive Director with evaluation of Horizon PACE LLC’s performance against national results and established service standards, and budget and other financial criteria.
  • Assists in development and implementation of performance improvement plans as indicated by evaluation of it.
  • Recommends changes in service delivery and staffing as required by the development and growth of the agency.
  • Participates regularly in all quality assurance/quality improvement committees as part of the overall quality improvement program.
  • In conjunction with the Executive Director and Quality Assurance Coordinator maintains the stability and reputation of Horizon PACE LLC’s program by insuring that all activities and operations are performed in compliance with local, state, and federal contracts, regulations, laws, accreditation, protocol, licensing and certification requirements governing Horizon PACE operations.
  • Establishes positive working relationships with health and social service providers.
  • Monitors Horizon PACE LLC’s operational compliance with Occupational Safety and Health Administration (OSHA) regulations and agency policies and procedures.
  • Protects privacy and maintains confidentiality of all company procedures, results, and information about, employees, participants, clients or families.
  • Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
  • Provides administrative on-call coverage on a rotation basis.
  • Performs related duties as required.

  • Is knowledgeable of participant rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all participants in a safe, secure environment.
  • Supports, cooperates with and implements specific procedures and programs
  • Supports and participates in common teamwork
  • Assist supervisor with various tasks to support department operations as directed; may also be assigned special projects.
  • Perform tasks which are supportive in nature to essential functions of the job, but which may be altered or re-designed depending on individual circumstances.
  • Assists participants and caregivers in filing grievances

Experience & Education:

  • Graduate of a school of professional nursing. Current Kentucky Registered Nurses License, Cardiopulmonary Resuscitation (CPR) certification required, Bachelors of Science Nursing (BSN) preferred.
  • A minimum of five (5) years of health care experience, and three (3) years of management experience, preferably in a geriatric care setting required. A minimum of one year’s experience in working with the frail or elderly population. Shall have either training or related experience in the job assigned.
  • Shall be knowledgeable of business administration and/or clinical practices. They shall be a professional in one of the following: Business, Nursing, Social Work, Psychology, Recreation Therapy, Occupational Therapy, Physical Therapy, Speech Therapy, Dietetics, or Gerontology.

Job Type: Full-time

Pay: $70,000.00 per year
