Position Summary
The Case Manager 1 is an entry level position responsible for providing case management with individuals and their families to establish appropriate linkages, referrals, coordination of care, and to provide regular follow-up on needed services and supports, under the supervision of a qualified Case Management supervisor. The Case Manager 1 will identify and assess client needs and evaluate, coordinate, and ensure necessary services are provided.
Required Education and Experience
- Bachelor’s degree from an accredited college or university in an applicable human services field (including social work, counseling, behavioral science, psychology, or a related field).
- No professional experience required – less than one year of human services experience. Prefer some relevant experience.
