What are the responsibilities and job description for the Care Coordinator position at Horizon Behavioral Health?
Hours: full-time (37.5 hours per week), Monday through Friday
Location: Bedford County Public Schools, Virginia
Position Summary
The Care Coordinator, QMHP-C or Qualified Mental Health Professional with child specialization, will serve as care coordinator in the school system, engaging all students, to meet the mental health needs within the school system. The Care Coordinator will engage, conduct initial screening, provide supportive level counseling and/or education, and refer students to a higher level of care. This position will be assigned to Bedford County public schools.
Required Education and Experience
- Bachelor's degree from an accredited college/university in an applicable human services field (including social work, counseling, behavioral science, psychology, or other DMAS approved human services and related fields degrees).
- One year of relevant clinical experience.

Related Knowledge, Skills and Abilities
Knowledge of psychotherapeutic interventions with education and mental health or substance abuse treatment. Demonstrate screening and assessment skills to determine need for higher level of care. Ability to engage with youth, school administration officials, and teachers in professional manner. Ability to articulate screening and assessment outcomes to parents, students, and administrators, and articulate next plan for client care.
Willing and available to work modified schedules (weekends, holidays, etc.) as required. Ability to drive and must possess a valid driver’s license. May be called upon to travel to any Horizon worksite; transport clients, and provide services to clients in other locations. Ability to participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed.
