As a Care Coordination Support Specialist at Horizon, you will…
- Update and maintain accurate assignments in Cerner, including Server and Assignment Category based on member’s level of care—status changes such as HARP and HH will be managed according to Standard timeframes to ensure up-to-date information is available for Supervisors and Program management.
- Maintain internal spreadsheets for member status, CCSS will coordinate with CC Supervisor for up-to-date information relating to HARP and HH status.
- Prepare mailings for members or care team, including Care Plan distribution, Notices of Determination for enrollment or discharge, housing or benefit applications, letters with updated team assignment and contact information, re-engagement letters, requested resources and any other information as requested by member or CC/CC Supervisor.
- Document all mailings in NetSmart, including uploading copies as determined by Standards.
- Upload signed Care Plans, consents or other documentation into NetSmart to support chart completion.
- Perform Gaps In Care, a billable Health Home core service, to include: contacting enrolled members to review Gaps in Care identified by their Managed Care Organization, providing education for routine Gaps in Care and assisting members with making appointments to address their Gaps in Care if requested by member, mailing educational information to members and documenting all activities in the NetSmart chart.
- Monitor Medicaid status for assigned team members and assist enrolled members with Marketplace recertification if needed.
- Assist enrolled members with setting up Medicaid transportation via MAS or Uber transportation via Horizon Uber account as needed.
- Request medical records at enrollment and annually for members on assigned care team, document requests and coordination to obtain with providers, and upload received records into NetSmart.
- Monitor Diligent Search Efforts status for assigned care team members, assist Care Coordinators with completing appropriate DSE activities.
- Audit NetSmart charts for appropriate, timely documentation and provide reminders to Care Coordinators for upcoming due dates.
- Attend any additional trainings as appropriate and directed by supervisor.

- Associate’s degree required.
- Care Coordination experience preferred.
- Excellent written and verbal communication skills.
- Proficient in Microsoft Office with expertise in Excel and Outlook required.
Location:
This is a full-time position located at 1000 Young St in Tonawanda, NY!
Hours:
This is a full-time position from Monday-Friday: 8:00am-5:00pm.
Compensation
Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $19.85 – 21.83. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.
Physical demands associated with this position include:
- Sitting for extended periods, alternating between standing and sitting
- Mobility required with repetitive wrist, hand, and finger movements
- Handling light duties, occasionally lifting objects up to 20 pounds
