As a Population Health Specialist at Horizon, you will…
- Monitor and follows up on key indicators identified in population health reports in collaboration with the business intelligence department.
- Participate in the development of standardizing work processes to impact key indicators which improve overall patient care.
- Support patients through specified outreach calls, telehealth, and face to face visits to identify barriers and supports patients referred for non-adherence, disease state management and social care needs.
- Offer support services to aid patients in improving medication adherence such as home delivery services, pharmacy coordination, medication packs, pill organizers, etc.
- Assist patients to achieve their goals of engagement in treatment and healthy lifestyle habits.
- Retrieve clinical and non-clinical data to assist in helping patients achieve goals via internal Electronic Health Record (EHR) and regional informational health systems, as appropriate.
- Provide support to patients relative to importance of following up with medical providers, as appropriate.
- Provide education and support to the behavioral health multidisciplinary team (prescribers, clinicians/counselors, care coordinators etc.) who refers patients with medication adherence and SDoH concerns.
- Participate in Population Health Team meetings.
- Attend case conferences to educate staff members on the medication adherence and population health services offered, in a continued effort to expand awareness.
- Travel between clinics to attend one on one sessions or attend virtual sessions, between patient and clinician, to discuss medication adherence interventions.
- Co-lead medication adherence and chronic disease groups (either onsite or virtual) as needed, with behavioral health clinician and/or prescriber.
- Attend off-site sessions in patient homes and in the community with behavioral health provider to re-engage patient in treatment services.
- Provide follow-up with patient’s post-hospitalization, regarding medication adherence or other health related issues.
- Participate in other projects and programming in regard to medication adherence or other population health related areas, as needed or requested.
- Have the ability to relate to individuals who have behavioral health disorders and their families of diverse ethnic, cultural, and socioeconomic backgrounds, and has the ability to interact with them in a positive, supportive, and cooperative manner.
- Have the ability to learn and apply Horizon Health Services Policy and Procedures and commit to adherence of privacy and confidentiality regulations.

We can’t wait to learn more about you! Here are a few specifics of what you’ll need for the job:
- Bachelor's degree required.
- OR -
- Associate's degree with 3 years of experience in the behavioral health field required.
- OR -
- High school diploma with 6 years of experience in the behavioral health field required.
- Must possess a clean NYS driver’s license and have reliable transportation
- Excellent computer skills, proficiency in Microsoft office skills
- Experience working within Electronic Health Record (EHR) systems and willingness to learn new software applications
- Familiarity with reporting and entry level data analytics
Location:
This position is specifically for our 2400 Pine Ave, Niagara Falls NY, frequent local travel is required.
Hours:
Monday through Friday day shifts, some flexibility to work evenings as needed.
Compensation
Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The hourly pay for this position is $19.20-22.44. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.
This position entails eligibility for a sign-on bonus of up to $1,500 for your commitment to work at our Horizon Certified Behavioral Health Clinics (CCBHC) located at our Pine Avenue Recovery Center and Lockport Recovery Centers for at least one year. A change in scheduled hours or location of employment may impact eligibility of bonus.
Physical demands associated with this position include:
- Sitting for extended periods, alternating between standing and sitting
- Mobility required with repetitive wrist, hand, and finger movements
- Handling light duties, occasionally lifting objects up to 20 pounds
- Local travel between worksites
- Exposure to various environmental conditions including low/high temperatures, outdoor elements
