As a Targeted Case Manager at Horizon, you will….
- Support patients through outreach calls, telehealth, and face to face visits to identify barriers and link patients to resources and services in the community.
- Provide transitional care, including appropriate follow-up outreach to connect patients from inpatient, emergency room, residential treatment to other settings and providers.
- Be an advocate for comprehensive care for the services needed as appropriate.
- Provide support to patients relative to importance of following up with behavioral health and medical providers, as appropriate.
- Offer support services to aid patients in navigating complex health care and social service systems.
- Retrieve clinical and non-clinical data to assist in helping patients achieve goals via internal Electronic Health Record (EHR) and regional informational health systems.
- Have the ability to relate to individuals who have behavioral health disorders and their families of diverse ethnic, cultural, and socioeconomic backgrounds, and has the ability to interact with them in a positive, supportive, and cooperative manner.
- Learn and apply Horizon Health Services Policy and Procedures and commit to adherence of privacy and confidentiality regulations.

What makes you a great candidate?
We can’t wait to learn more about you! Here are a few specifics of what you’ll need for the job:
- Master’s degree in health and human services preferred.
- Bachelor’s degree required
- One-year prior relevant experience preferred.
- Computer navigation skills, proficiency in Microsoft office skills.
- Experience working within Electronic Health Record (EHR) systems and willingness to learn new software applications preferred.
- Familiarity with reporting and entry level data analytics.
Location:
This position is specifically for our 3020 Bailey Ave clinic in Buffalo NY, frequent local travel is required.
Hours:
Monday through Friday day shifts, some flexibility to work evenings as needed.
Compensation:
Horizon is consistently evaluating our compensation and benefit packages to ensure we remain competitive. The range of pay for this position is $20.00 - $24.00. Starting pay may be impacted based on experience and education. Compensation adjustments are evaluated annually.
Physical demands associated with this position include:
- Sitting for extended periods, alternating between standing and sitting
- Mobility required with repetitive wrist, hand, and finger movements
- Handling light duties, occasionally lifting objects up to 20 pounds
- Local travel between worksites
- Exposure to various environmental conditions including low/high temperatures, outdoor elements
