Our Home Care Kenosh Team is currently seeking an Onsite Clinical Coordinator to ensure appropriate staffing and coordination for all team patients. Coordinates the daily Clinical Team functions from the office to assure effective coverage of all Team clients'. Acts as a resource for field staff, including Therapist, Case Managers, Patient Care Coordinators, Nurses and Home Health Aides while functioning as a liaison that represents the team with all internal and external customers. Provides services at quality levels consistent with professional standards of practice.Participates in holiday office coverage rotation.
This vital position will:
