The role is a hands-on case manager with a 4-10 schedule, Wednesday-Saturday 0830-1900
Responsibilities:
- Provide comprehensive nursing care to patients and families in a hospice or palliative care setting.
- Assess patient needs and develop individualized plans of care.
- Develop and implement strategies for symptom management and end-of-life care.
- Coordinate with other healthcare professionals to ensure continuity of care.
- Educate patients and families about the disease process, treatment options, and available resources.
- Provide emotional support to patients and families throughout the course of their illness.
- Document patient care activities in accordance with established standards.
- This position is a hands-on care, case manager who will be responsible for providing care to patients and families in a home setting.

QUALIFICATIONS:
Current RN license in the State of Washington
Two years clinical/home care experience preferred
Skilled in nursing practice, communication and commitment to Hospice Philosophy
Must have approved Criminal Background check
Must successfully complete performance evaluation and a nursing skills competency evaluation during probationary period and annually thereafter.
Valid Drivers License and active Auto Insurance Card.
