Horizon PACE is excited to announce an amazing opportunity to be part of Horizon PACE’s program (Program of All-Inclusive Care for the Elderly). This innovative program is for individuals 55 years or older, who are living with chronic illness, or disabilities and need coordinated medical care to continue living as independently as possible in their home and community. The healthcare team at Horizon PACE will approach coordinating medical, social, and home care services so individuals no longer need to manage their medical care alone. Horizon has over 25 years’ experience providing care in Kentucky and is the largest provider of adult day health care services in the state. Horizon is opening 3 PACE locations in Kentucky, and we are excited to expand services to older Kentuckians. Under the supervision of the Horizon PACE Director, the Transportation Coordinator is responsible for the Horizon PACE transportation program, including transport of participants, meals, durable medical equipment, pharmacy, and any other transportation as deemed necessary. Responsible for hiring, training and supervision of all drivers and dispatchers. Oversees vehicle repairs and maintenance. Responsible for meeting departmental budget goals.

Position Responsibilities:

  • Supervises the transportation departmental staff. Responsible for departmental hiring, orientation, competency, and training procedures.
  • Functions as a member of the Interdisciplinary Team (IDT). Maintains regular attendance at, and participants in Interdisciplinary Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery.
  • Communicates Interdisciplinary Team decisions, participant changes, plan of care changes, and updated information to transportation staff daily.

· Ensures responsibility for running and maintaining transportation scheduling software program.

· Ensures effective daily communications, via calls or otherwise, between PACE Center, transportation drivers, dispatch and Transportation Coordinator.

· Ensures the scheduling, coordination, communication of the driver route assignments of Horizon PACE.

· Ensures the scheduling of transportation for all participants outside appointments.

· Monitors use of Driver/Dispatch overtime, ensure adequate staffing and improves efficiency.

· Ensures the daily work schedules for each Transportation Associate/Driver is communicated to each employee.

· Ensures that Horizon PACE Centers receive in a timely manner the daily work schedules for each Transportation Driver.

· Ensures coordinating and scheduling daily delivery of home delivered meals, Durable Medical Equipment (DME), supplies and miscellaneous items as requested by the Horizon PACE IDT.

  • Ensures that participant’s database is maintained and updated.
  • Collaborates with the Center Manager in the annual and capital budget development.
  • Maintains the departmental budget within its allowed and reasonable objectives as well as meets established budgetary guidelines
  • Oversees vehicle maintenance program and obtains estimates and authorizes vehicle repairs per departmental policies or established guidelines.
  • Research new vehicle options and assists in the ordering and purchasing of new vehicles.
  • Ensures that vehicles have up-to-date Department of Motor Vehicle registration forms, license plates. emission stickers, etc.
  • Ensures cleanliness, proper and functioning equipment as well as of safe driving conditions of each vehicle.
  • Ensures that all departmental staff attends and successfully executes required annual driver training courses and participates in mandatory in-service training.
  • Sets clear expectations and assigns appropriate authority to staff; holds staff accountable for their scope of work and compliance to company policies and procedures.
  • Follows all Horizon PACE policies and procedures and any applicable Occupational Safety and Health Administration (OSHA) safety guidelines.
  • Maintains the confidentiality of all company procedures, results, and information about participants, clients or families.
  • Handles potentially infectious specimens with appropriate biohazard precautions, and practices Universal Precautions.
  • Participates in and supports Quality Improvement Initiatives.
  • Participates in continuing education classes and any required staff and training meetings. Maintains professional affiliations and any required certifications.
  • Performs other duties as required or requested.

· Is knowledgeable of participant rights and ensures an atmosphere which allows for the privacy, dignity and well-being of all participants in a safe, secure environment

· Supports, cooperates with and implements specific procedures and programs

· Supports and participates in common teamwork

· Assist supervisor with various tasks to support department operations as directed; may also be assigned special projects

· Perform tasks which are supportive in nature to essential functions of the job, but which may be altered or re-designed depending on individual circumstances

· Assists participants and caregivers in filing grievances

· Knowledge of safety requirements pertaining to the transportation industry.

· Requires individual who can coordinate multiple tasks simultaneously.

· Skilled in establishing and maintaining effective working relationships with participants, co-workers, medical staff, and the public.

· Proficient in identifying and recommending problem resolution.

· Ability to work independently with minimum supervision.

· Ability to manage a transportation scheduling computer software.

· Familiar with operation of accessible vehicles including wheelchair (w/c) lifts, wheelchair tie downs and safety equipment.

· Experience with maintaining and oversight of a fleet of vehicles

Experience & Education:

· High school diploma/equivalent preferred; college degree preferred. Valid Kentucky Driver’s License

· Preferred to have the following:

o Two (2) years of administrative/supervisory experience, in a transportation setting.

o Five (5) years of experience in the transportation industry.

o Have (1) year experience in working with the frail or elderly population.

Pay: $22.00 per hour
