Our office is located in Temecula, CA and we are seeking nurses and therapists who can cover the following areas:
- Valley Center, CA
- Pomona, CA
- Julian, CA
- Pauma Valley, CA
Please note, all employees are required to attend quarterly office meetings, in Temecula, as well as attend orientation and pick up supplies at our office.
We are seeking both Licensed Vocational Nurses and Registered Nurses, as well as Physical Therapists and Physical Therapist Assistants to join our field team.
LVN Responsibilities
- Provides direct patient care as defined in State’s Name Nurse Practice Act.
- Implements plan of care initiated by the registered nurse.
- Provides accurate and timely documentation consistent with the plan of care.
- Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care
- Participates in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor/Nursing Supervisor.
RN Responsibilities
- Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
- Regularly re-evaluates patient nursing needs.
- Initiates the plan of care and makes necessary revisions as patient status and needs change.
- Uses health assessment data to determine nursing diagnosis.
- Develops a care plan, which establishes goals based on nursing diagnosis and
- incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.
- Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
- Counsels the patient and family in meeting nursing and related needs.
- Provides health care instructions to the patient as appropriate per assessment and plan of care.
- Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
PT Responsibilities
- Provides physical therapy services to patients according to a written physician's plan of care.
- Initiates physical therapy program and instructs other personnel and/or family/caregiver members in certain phases of physical therapy with which they may work with a patient, as well as instructing them as to the goals of the physical therapy program for the patient by participating in case conferences.
- May train patient in the use of prosthetic device.
- Identifies patient and family/caregiver needs for other home health services and refers as necessary.
- Prepares and submits clinical and progress summaries based on the attainment of goals.
- Participates in discharge planning for patient.
PTA Responsibilities
- Provides direct physical therapy according to directions of physical therapist and in accordance with the physician's plan of care.
- Treats patient to relieve pain, develop or restore function, and maintain maximum performance.
- Directs and aids patients in active and passive therapeutic exercises, muscle reeducation, gait and functional training, transfer activities and prosthetic training.
- Utilizes physical agents, including, but not be limited to heat, cold, water, light, electricity, ultrasound and massage.
- Observes and reports to the physical therapist and the Organization personnel the patient’s reaction to treatment and any changes in patient’s condition.
- Instructs patients in care and use of wheelchairs, braces, crutches, canes, and prosthetic or orthotic devices.
- Maintains necessary records; clinical notes and conference notes, which will be incorporated into the patient’s clinical record in prescribed time frames.
- Participates in Organization Home Care Team Members meetings and Organization Inservice Education Program.
- Performs other duties as assigned.
Qualifications
- Currently licensed as a licensed practical nurse or licensed vocational nurse in California
- Currently licensed as a registered nurse in California
- Licensed to practice as physical therapist within California
- Maintains current CPR
- Must be a licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.
