About the Position
We are seeking a skilled and compassionate Licensed Clinical Social Worker to join our team at Horizons Health Care Center. As a member of our interdisciplinary team, you will play a critical role in supporting the care and treatment of residents entrusted to our facility.
Key Responsibilities
- Provide direct care to residents, including assessment, care planning, and coordination of care.
- Lead transfer and discharge teams to ensure safe and efficient transitions for residents.
- Maintain accurate and detailed records of resident interactions, assessments, and care plans.
- Communicate effectively with residents, families, and healthcare professionals to ensure seamless coordination of care.
Requirements and Qualifications
- Bachelor's degree in social work or related field.
- Licensed Clinical Social Worker (LCSW) certification in the state of Colorado.
- At least one year of experience in long-term healthcare settings, preferably in a similar role.
- Familiarity with Federal and State regulations and requirements for long-term care.
Compensation and Benefits
- Competitive hourly rate: $28.85 - $35.58 per hour.
- Part-time schedule with days and times varying, occasional weekends.
