What are the responsibilities and job description for the Adult Day Service Assistant/Driver position at Horizons of Tuscarawas and Carroll Counties, Inc.?
Basic Function :
The Adult Day Service Assistant will transport DD Individuals from our Bolivar Ohio ICF Facility to our Adult Day Facility in New Philadelphia Ohio and community outings, you will also work directly with developmentally and intellectually disabled individuals in groups and one-on-one to help meet their various social, physical, vocational and psychological goals, needs and desires.
Primary Responsibilities:
Provide direct care and habilitation programming to individuals.
Maintain adequate personal hygiene of individuals by assisting with grooming, dressing and undressing, feeding, toileting and positioning.
Must be able to twist, bend, stoop and kneel.
Must be able to lift and/or carry up to 50lbs.
Must be able to assist other with lifting adults.
If an individual resides in a residential facility, promptly report to the nurse or Manager any suspected need for medical attention or individual changes.
Serve as chief resource person for the day-to-day status and needs of the individuals.
Work with the professional staff to evaluate assigned individuals' present abilities, progress and potential.
Participate in the development and follow-through of assigned individuals' IP's.
Follow and document the programs for each individual.
Properly use and care for each individual's adaptive and personal equipment: i.e., wheelchair, eating utensils.
Interact with each individual and co-worker in a manner which is consistent with respect for the individual and his value as a human being.
Interact physically with each individual and co-worker in a socially and professionally acceptable manner.
Use acceptable language and tones of voice.
Maintain a safe, clean, healthful environment for individuals and co-workers, including laundry.
Assist individuals in the community as needed, to work toward personal goals, dreams or desires.
Assist individuals with vocational training when assigned.
Participate in on-going staff development activities.
Must be available to work flexible hours as needed in order to complete job duties.
Follow Horizons, Inc. policies and procedures in compliance with HIPAA Privacy Rule.
Will complete Orientation Program.
Complete CPR, First Aid classes, NCI Training and Delegated Nursing class.
Other related duties.
