Overview
We are seeking a dedicated and compassionate Habilitation Program Monitor/Recreation Specialist to join our team. The ideal candidate will monitor that active treatment is provided at the facility on an on -going basis. To process and practice a commitment to provide opportunities in the community for the Individuals residing in the home. The Habilitation Program Monitor/Recreation Specialist will work directly with the Individuals both individually and in groups to help meet their various social, physical and psychological needs at home and in the community.
Responsibilities :
- Observe the direct support staff as they assist the individuals in the home and community settings, assisting with daily living activities.
- Train staff on active treatment and monitor that active treatment is a part of all areas of the individuals lives.
- Schedule transportation and accompany individuals on community outings.
- Schedula and document monthly meetings with the individuals of each home to ascertain what community activities they would like to attend.
- Provide direct care and habilitation programming to individuals.
- Maintain a safe, respectful, and nurturing environment for all individuals served.
Qualifications:
- High School Diploma or GED are required for this position.
- Must have evidence of the ability to work constructively with developmentally disabled individuals.
- Previous direct care,nursing and /or developmental disabilities experience preferred.
- Must have a valid drivers license.
- Must possess good general health and be able to lift or help lift adults..
- Will be required to transport individuals for activities,doctor appointmnets,etc in a 12 passenger van with a wheelchair lift.
- Must be able to work flexible hours as needed to accommodate individuals needs and schedules.
