Job Overview
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team.
Qualifications:
One must be a graduate of an approved school of nursing and must have pharmacology. Must have current registration in this state. Six months experience in nursing preferred. Previous experience with handicapped persons desirable. Good physical and mental health a must. Must have sincere desire to work with handicapped persons. Must be willing to perform non-traditional duties. Must be well-groomed, courteous, tactful, patient, kind and pleasant when dealing with individuals. Must be level-headed in emergency situations. Must have acute sense of responsibility. Must possess a spirit of cooperation and enthusiasm. Must have a valid driver's license.
- Provide professional nursing service for individuals as indicated by the Individual Plan, based on established policies and procedures.
- May accept, transcribe, and initiate telephone orders from the Medical Director.
- Complete medical treatments ordered by the Medical Director.
- Complete daily nursing documentation as required in this position, and assess any individual needs.
- Continue professional growth by attending inservices, workshops, etc.
- Order, safely administer, and accurately record medications given (ICF/IID facilities).
- Make nursing assessments of ill individuals, follow standing orders, and notify physician if necessary.
- File paperwork for individuals as needed.
- Complete tube feedings as scheduled and ordered.
- Complete colostomy care as scheduled and ordered.
- Order medications as needed.
- Make doctors’ appointments when necessary on your days of work
- Complete Incident/Injury Reports as required.
- Implement individuals’ self-administration of medication goals and document accordingly.
- Other related duties.
Duties
- Provide direct patient care under the supervision of registered nurses and physicians.
- Assist with daily living activities and personal hygiene for patients.
- Monitor patients' vital signs and report any changes to the healthcare team.
- Administer medications and treatments as prescribed.
- Participate in discharge planning to ensure smooth transitions for patients leaving the facility.
- Maintain accurate patient records using electronic health record systems such as Epic or eClinicalWorks.
- Collaborate with interdisciplinary teams to develop and implement patient care plans.
- Educate patients and their families about health management and available resources.
- Ensure compliance with safety protocols and infection control measures within the facility.
Requirements
- Valid Licensed Practical Nurse (LPN) license in the state of practice.
- Experience in inpatient settings, occupational health, or long-term care preferred.
- Familiarity with medical equipment and durable medical equipment (DME).
- Strong communication skills to effectively interact with patients, families, and healthcare professionals.
- Ability to work collaboratively within a team environment while also being self-motivated.
- Knowledge of electronic health record systems such as Epic or eClinicalWorks is a plus.
- Compassionate approach to patient care with a focus on enhancing quality of life.
- Previous experience in assisted living or discharge planning is advantageous.
