As a valued care team member, you’ll play a vital role in creating a supportive and enriching environment for our residents.
Here’s how you’ll contribute:
- Be a Care Partner: Provide compassionate assistance and guidance with daily living activities, such as dressing, grooming, and mealtimes, fostering a sense of dignity and self-reliance.
- Facilitate Engaging Experiences: Engage our residents in meaningful activities according to their individual programs and group experiences to promote their well-being.
- Help Build Skills: Guide residents in building new skills, maintaining existing abilities, and actively participating in community inclusion activities.
- Be a Positive Role Model: Use positive reinforcement to motivate and encourage residents and fellow staff members.
- Be a Safety Advocate: Proactively address any concerns and adhere to Horizons’ policies and procedures to ensure a safe and healthy environment.
- Be a Team Player: Foster a collaborative and supportive atmosphere by communicating effectively with colleagues and contributing to conflict resolution.
- Be a Problem Solver: Confidently handle unexpected situations, making sound decisions in emergencies and escalating concerns to supervisors when needed.
- Be a Documentation Specialist: Accurate and timely documentation is vital to ensuring the continuity of care for our residents. Your detailed records will directly contribute to their well-being and progress.
What you’ll bring to the Team:

- The ability to lift, push, pull, and move 40 pounds to assist with resident mobility.

DSPs must be at least 18 years old and have a high school diploma or equivalent.
