Key Responsibilities
- Responsible for making face to face visits with patients or families in the hospice program
- Attend bi-weekly IDG meetings
- Complete hospice assessments related to the care plan
- Is responsible for the hospice bereavement program
- Participates in agency and community programs such as Hospice Tree of Lights and Living Through Loss
- Education: Must have one of the following:
- a Masters of Social Work degree from a school accredited by the Council on Social Work Education and one year of experience in a health care setting;
- OR have a Bachelors degree in Social Work from an accredited institution by the Council on Social Work Education
- OR a Bachelors degree in psychology, sociology, or other field related to social work with at least one year of experience
