Hiring a part time Social Worker / Bereavement Coordinator to work in the Community Health Department. This position is 16 hours in a two-week period.
Key Responsibilities
- Responsible for making face to face visits with patients or families in the hospice program
- Attend bi-weekly IDG meetings
- Complete hospice assessments related to the care plan
- Is responsible for the hospice bereavement program
- Participates in agency and community programs such as Hospice Tree of Lights and Living Through Loss
Qualifications :
a Masters of Social Work degree from a school accredited by the Council on Social Work Education and one year of experience in a health care setting;
Position is 16 hours in a two-week period. Working hours are Monday through Friday, 8 : 00am to 4 : 30pm. Days may fluctuate based on patient need. Applicant must pass background check, drug screen and health assessment including applicable vaccinations prior to hire. Position open until filled.
