RN Case Manager - Canadian Valley - Days

We have an exciting opportunity for a Full-time RN Case Manager in Yukon, OK, working 8a-4:30p with our Case Management team. This role involves managing clinical and financial outcomes for assigned patients, coordinating care, and facilitating timely discharges. Responsibilities include:

• Conducting comprehensive patient assessments.
• Coordinating transfers and discharge plans.
• Communicating care plans to patients and healthcare professionals.
• Monitoring resource utilization and compliance with regulations.
• Collaborating with various departments to ensure efficient patient care.
  
  

Applicants should have 5 years of clinical experience, current RN licensure in Oklahoma, and strong communication skills. A BSN and Case Management Certification are preferred.
