Monday through Friday with on-call rotation
Covers the Bowling Green and Madisonville areas (and surrounding counties)
Nurse Practitioner Position Summary:
The palliative care nurse practitioner (NP) is assigned to the Hosparus Health palliative care community and reports to the Senior Nurse Practitioner. The NP provides direct patient care, including making medically-necessary palliative care visits (billable) and hospice face-to-face recertification (FTF) visits (non-billable). The NP will also rotate to cover general Hosparus Health-wide weekday and weekend call for orders and prescriptions. In addition, the NP supports and coordinates care with the Interdisciplinary Group (IDG) and participates in education, outreach and leadership activities for the community location. The NP may be assigned additional patient care or administrative duties at the discretion of the Senior Director of Clinical Excellence or SVP/Chief Medical Officer.

Nurse Practitioner Position Qualifications:
- MSN or higher-level Adult Nurse Practitioner required.
- Licensure (or eligibility for licensure) for advanced nursing practice in both KY and IN, including DEA and controlled substance permits, required.
- Minimum of one year experience in full time advance nursing practice preferred.
- Primary care, palliative care, hospital-based, or hospice experience preferred.
- Fulfill requirements of on-call duty policies.
- Must have a valid driver’s license in accordance to policy.
- Use of technology-based tools, including electronic medical record, strongly preferred.
