Overview
The Care Coordinator at Hospeace House is a part-time leadership role essential for providing comprehensive, compassionate comfort care to one or two hospice residents who are living in our home as they complete their life's journey. Working collaboratively with the Operations Director, per diem nurses, volunteers, paid staff, as well as resident families, this position ensures dignity and quality of life for residents at the end of life.
Responsibilities
- Intake Assessment: Evaluate referrals for admission, assess care needs, and guide families through the admission process.
- Resident Admissions: Conduct clinical assessments, prepare medications, and review protocols with families and staff.
- Daily Resident Care Oversight: Monitor resident comfort and safety, address clinical changes, and provide hands-on care as needed.
- Leadership Duties: Train and supervise per diem nurses, develop care protocols. In close collaboration with the Operations Director, develop volunteer and paid staff training program.
Skills Needed
- Active RN, LPN, EMT, or paramedic license in New York State.
- 10 years of medical experience, preferably in home care or end-of-life care.
- Strong leadership, communication, and organizational skills.
- Ability to lift 35 lbs and perform physical tasks related to resident care.

Pay: $30.00 - $35.00 per hour
Expected hours: 20 – 25 per week
