Our team is looking for a full-time Hospice RN Case Manager in Worcester County!

As a case manager, you direct the hospice care of our patients for our agency.
Requirements :
- RN license in Massachusetts, active and unencumbered.
- Bachelor's in nursing from an accredited program.
- Prior hospice experience is preferred, but not required.
- CPR certification.
