What are the responsibilities and job description for the Bereavement Coordinator position at Hospice and Francis?
Under the general direction of the Chief Operations Officer, the Bereavement Coordinator is responsible for implementing, managing, supervising and coordinating the full range of comprehensive grief support services to families and caregivers of hospice patients. The Bereavement Coordinator provides support via telephone, mail, and in person through individual grief support, memorial services, grief workshops, family assessment and conference. Provides therapeutic counseling to family members.
EDUCATION / TRAIN ING REQUIRED :
Master's Degree in Mental Health Therapeutic Counseling or Marriage and Family Counseling.
CERTIFICATION / LICENSE : Valid Florida Driver's License. Licensed in Mental Health Counseling or Marriage and Family Counseling is required.
EXPERIENCE / SKILLS / ABILITIES :
Ability to work flexible hours. Experience in counseling and / or work with aging. Must be able to work independently and in an interdisciplinary environment. Bereavement training preferred.
PHYSICAL REQUIREMENTS :
Able to access patient homes and meeting locations including those without elevators. Able to work under varying levels of stress. Able to lift, push, pull twenty-five (25) pounds without difficulty. Good audio and visual acuity. Able to work in various facility and home environments including those with exposure to tobacco smoke and pets.
PRIMARY RESPONSIBILITIES :
- Ensure departmental compliance with all Medicare / Medicaid, Federal, state, NHPCO and CHAP regulatory requirements relating to bereavement services.
- Provide coordination of bereavement support and services to family and caregivers hospice patients. Provides therapeutic counseling to family members.
- Coordinates contact with the bereaved within appropriate time frames as indicated on the bereavement assessment and bereavement plan of care.
- Evaluate survivor's grief support needs and develop individualized care plan.
- Maintain accurate and quality documentation of all bereavement activities within specified time frames in SFR designated electronic health record.
- Acts as a consultant / resource person to the Interdisciplinary Team (IDT) members regarding pre-bereavement and bereavement.
- Provides education on topics related to grief and bereavement, including assistance with new employee and volunteer orientation.
- Organizes and facilitates memorial services and bereavement special programs for survivors.
- Organizes and facilitates grief support groups on that support the needs of the community.
- Develops, revises and maintains bereavement policies and procedures in conjunction with the COO and Compliance Officer.
- Participate in performance improvement activities as needed.
- Responsible for knowledge in all areas of theories of grief and loss, appropriate grief interventions, crisis interventions, group dynamics, and systems theories.
