Summary: Provides social work services for patients and families served by the hospice inpatient unit.
Qualifications:
- Bachelors or Master’s degree from an approved school of social work
- Current South Carolina license
- Experience in a health care setting preferred
- Ability to clearly explain Hospice services including reimbursement issues is required.
Responsibilities | Job Duties:
- Assesses and identifies social, emotional and financial needs of patients and families served by organization programs.
- Participates in establishing and evaluating the Interdisciplinary Plan of Care.
- Makes patient visits, according to organizational productivity standards, and maintains ongoing counseling and support for patients and families.
- Provides counseling to patients and families regarding decisions for Advanced Directives and other financial and legal matters.
- Serves as resource for the clinical team. Assists members in understanding family dynamics in times of stress and crisis.
- Provides resource counseling by providing referrals to community service agencies which may benefit the patient and family as appropriate.
- Participates in volunteer training and community education on the Hospice philosophy, needs of dying, loss, and grief.
- Participates as a member of the Bereavement Team. Provides grief assessment, support and counseling to appropriate individuals and groups as assigned.
- Supports the Bereavement Coordinator by assisting with program activities.
- Available for support of hospice staff and volunteers as requested
- Participates in planning and staffing of Project Hope activities including school groups and bereavement camp.
- Documents, including electronic documentation, all aspects of care and services provided according to organizational policies.
- Supports the overall culture of the organization
- Participates in organization sponsored events.
- Performs other duties as requested and appropriate.

Pay: $28.00 - $30.00 per hour
