ESSENTIAL DUTIES AND RESPONSIBILITIES

- Assist physicians and other Hospice care staff and interdisciplinary team members in understanding the significant spiritual factors related to client physical health, mental health, and spiritual issues.
- Participate in the development of client’s plan of care and participate in case conferences with agency staff.
- Prepare client clinical and progress notes, incorporating notes into the clinical record.
- A chaplain will be open, sensitive, flexible and ecumenical, with working with patients, families and/or significant others of the patient.
- A chaplain will possess active listening and communication skills.
- A chaplain will possess the ability to assess the patient, family and/or significant other’s spiritual concerns and needs.
- A chaplain will provide direct spiritual support and/or counseling to patients, their families and/or significant others in keeping with each individual belief system.
- The chaplain will communicate with appropriate clergy and spiritual counselors within the agency’s service area.
- The chaplain will maintain proper records of visits to patients, families and significant others of those we serve.
- The chaplain will perform occasional liturgical assignments such as Memorial Services or Services of Remembrance and reflections during Interdisciplinary Team Meetings.
- In rare instances, the chaplain may be asked to assist with or conduct funeral or memorial services as appropriate after consulting with her or his immediate supervisor or designee.
- The chaplain will maintain a resource group of clergy, who have specific areas of spiritual expertise. As appropriate, patient, family and/or significant other’s spiritual care needs may be delegated.
- The chaplain will provide educational programs for community clergy, religious and lay representatives as appropriate and as the agency resources allow.
- Chaplains will maintain competencies of:
*end-of-life care philosophy;
*responding to persons of differing faiths;
*alertness to signs of abuse, neglect and exploitation; and
*alertness to signs of suicidal or homicidal ideation.

Requirements
QUALIFICATIONS
- Prefer seminary or school of theology graduate or have appropriate certification in pastoral education.
- Ability to effectively problem solve and possess decision-making skills.
- Exceptional verbal communication and writing skills.
- Ability to work with staff to meet the psychosocial, emotional, and spiritual needs of the patient/family.
- Demonstrate ability to be open, sensitive, flexible, and ecumenical.
