Description:
Hospice By The Sea provides end-of-life care for patients with life-limiting illnesses. We work with the patients and families concurrently to provide comfort and relief from symptoms experienced during a chronic long term illness. This role covers mostly North County and Central San Diego.
Hospice By The Sea is currently seeking Hospice Registered Nurses and Hospice On-Call Registered Nurses.
Duties Include:
- Completing initial and ongoing comprehensive assessments of patient and family to determine hospice needs.
- Provide professional nursing care.
- Initiate the plan of care and make necessary revisions as patient status and needs change.
- Develop a care plan that establishes goals, based on the nursing diagnosis and incorporates palliative nursing actions - includes the patient and the family in the planning process.
- Counsel the patient and family in meeting nursing and related needs.
- Meet weekly with IDT members
RequirementsQualifications :
- Graduate of an accredited school of nursing. One plus years' of recent Hospice experience.
- Current licensure in State and valid driver’s license.• One year of hospice experience preferred.
- Excellent observation, verbal and written communication skills,

Pay: $45.00 - $60.00 per hour
