Job Summary
Manages the hospice Inpatient Center (IPC), responsible for overall clinical operations, ensuring the delivery of exceptional end-of-life patient care, and fostering a collaborative and supportive work environment for the interdisciplinary team. Addresses the ongoing needs of patients and caregivers, supporting and enhancing the organization mission and philosophy, and ensuring compliance with all regulatory standards.
Essential Functions
Clinical Leadership and Management
Provides clinical leadership and direction to all clinical staff within the inpatient center
Manages interdisciplinary (IDT) direct care services in accordance with regulatory requirements, Joint Commission standards, and organization policy and procedure.
Assures efficient 24-hour staff scheduling and utilization of team resources.
Works directly with hospice physicians for effective integration of protocols and medical services within the IDT.
Facilitates or delegates responsibility for IDT meetings assuring appropriate review of patient plans of care in accordance with regulatory requirements, standards and CMS Conditions of Participation
Assures accurate and timely documentation of clinical care.
Serves as a clinical resource to clinical staff, home hospice Team Leaders and clinical support staff.
Coordinates utilization of indirect and ancillary services (DME, infusions, pharmaceuticals, outpatient services, therapies, etc.)
Ensure the safe administration of medications and treatments.
Assists with assessment of staffing needs, development of staffing recommendations and with the interview and hiring process for clinical staff.
Assures regular and timely evaluation of staff, including disciplinary actions.
Maintains oversight of private insurance benefits, current records and assisting to provide timely updates to insurance companies as required.
Honors and respects the customs, choices, needs and goals of each patient and family.
Mentors shift supervisors to foster growth in organizational and managerial skills.
Compliance and Quality Assurance
Ensure compliance with all federal, state, and local regulations, as well as hospice policies and procedures.
Participates in utilization review, assists with quality opportunities, and strategic planning and other committees as assigned.
Reviews and recommends inpatient policy and procedure.
Communication and Collaboration
Facilitates communication for coordination of care between admissions, home hospice and inpatient staff, collaborating with other teams to affect a smooth transfer process to and from the IPC.
Collaborates with Clinical Directors, the Leadership Team, and Education Staff for the development, delivery, and evaluation of clinical educational activities.
Collaborates with Clinical Directors and the Leadership Team for the review, development and implementation of clinical procedures.
Communicates and collaborates effectively with patients, families, physicians, and organizational staff, outside contractors and community agencies.
Models effective methods of collaboration and coordination of direct patient care.
Fiscal Responsibility
Assist in the development and management of the nursing department budget for the inpatient center.
Understands and interprets reimbursement and payer source implications to the plan of care, insuring wise and effective use of resources.
Monitor resource utilization and ensure cost-effective practices.
General Organization Functions
Acts as a role model by demonstrating a positive professional demeanor in a nurturing, supportive manner.
Assures effective orientation of all clinical staff to the Inpatient Center.
Assists with orientation, volunteer training and community education as requested.
Qualifications
Registered nurse licensed in Pennsylvania
BSN preferred
Minimum of two (2) years recent nursing experience required
Hospice, oncology, and/or public health experience preferred
