This position will support patients in the following cities: Vallejo, Hercules, Richmond, Emeryville, Oakland and surrounding areas.

Position Summary
Reporting to the Team Manager, the RN Case Manager assesses and addresses the physical, psychosocial, environmental, and developmental needs of patients and families. This role involves coordinating and implementing individualized care plans in collaboration with the attending physician, Medical Director, and interdisciplinary team. Each Case Manager oversees the care of up to 15 patients with the support of Revisit Nurses and the Team Manager.

Key Responsibilities

  • Identify and address signs of active dying and patient transitioning.
  • Assess and manage patient pain and symptoms, ensuring individualized care plans.
  • Educate and support patients, families, and caregivers using teach-back methods.
  • Maintain accurate and timely documentation of assessments, interventions, and patient outcomes.
  • Provide holistic, family-centered care across various settings to enhance quality of life.
  • Respect and support patients' spiritual and cultural beliefs.
  • Collaborate with the interdisciplinary team and external care providers.
  • Coordinate patient care, considering insurance benefits, authorizations, and cost containment.
  • Facilitate care transitions to General Inpatient (GIP) or respite levels as needed.
  • Supervise Licensed Vocational Nurses (LVNs) and Home Health Aides.
  • Participate in interdisciplinary team meetings, staff education, and quality improvement initiatives.
  • Ensure compliance with state, federal, and accreditation standards.

Position Requirements

Must have the ability to perform a complete nursing physical assessment, develop a comprehensive care plan and respond to the needs of patients and families in the home setting, which includes SNF, RCFE/Board & Care. Must be able to cope with patient/family emotional stress and be tolerant of individual lifestyles without judgment. Must have ability to establish professional boundaries and maintain cooperative relationships with professionals. Must demonstrate excellent interpersonal communication skills, customer service skills, initiative, dependability and good judgment. Must be able to work collaboratively as part of an interdisciplinary team, as well as independently, and problem solve in a crisis situation. Must take (and pass with 85% or more) a medication calculation competency exam annually. Demonstrates clinical competency as observed during supervisory visit.

Qualifications & Requirements

  • Current California Registered Nurse License.
  • Experience in Nursing.
  • CPR certification.
  • Valid California driver’s license and a reliable vehicle.
  • Hospice or home care experience preferred.
  • Certification in Hospice and Palliative Care Nursing (highly desirable).
  • IV therapy experience is a plus.
  • Proficiency in basic computer skills.
