Work Schedule: Tuesday, Wednesday, Thursday 8am-5pm
POSITION SUMMARY:
This position reports to the VP Chief Clinical Services Officer. As a palliative care social worker, responsibilities include providing agency patients and families with assessment, evaluation, counseling and casework to address social, economic, and psychological needs. Social worker will assist in coordinating care with other healthcare providers as directed by the patient and other Palliative Care team members.
ESSENTIAL FUNCTIONS:
- Completes initial as well as ongoing assessments of the psychosocial needs, e.g., suicide, bereavement, etc., of all assigned patients/families.
- Develops plan of care for patient/family that is tailored to individual needs and includes the interdisciplinary team as appropriate.
- Reports and documents patient/family contacts and interventions to other members of the interdisciplinary team appropriately.
- Attends and participates in interdisciplinary team meetings, staff meetings, and other meetings as indicated.
- Provides interventions as indicated in the interdisciplinary team care plan.
- Identifies and facilitates referrals to appropriate community resources and coordinates patient and family use of these resources.
- Assists with benefit programs and end of life planning.
- Develops and/or participates in education programs and workshops for staff and community at large.
- Provides input to the Social Services Manager as to the ongoing clinical needs of the program as well as issues of quality improvement.
- Adheres to federal and state regulations, as well as accreditation standards.
- Performs other duties as assigned.
POSITION REQUIREMENTS:
- Excellent interpersonal and communication skills.
- Understanding of the psychosocial issues related to families in crisis and end-of-life issues.
- Ability to perform a psychosocial assessment and intervene appropriately.
- Ability to work effectively with individuals and groups.
- Ability to work in the home setting or nursing home setting.
- Must be able to handle multiple tasks, be flexible and prioritize work load.
- Must be highly organized
- Ability to provide culturally sensitive support to people from diverse ethnic, cultural, and socioeconomic backgrounds.
- Must be able to work well under pressure meeting multiple and sometimes competing deadlines.
- Shall, at all times, demonstrate cooperative behavior with colleagues and supervisors.
EXPERIENCE AND EDUCATION:
- Master of Social Work degree from a school accredited by the Council on Social Work Education
- One year social work experience in a health care setting preferred; experience in Long Term Care setting a plus
- Clinical experience with geriatric, AIDS, oncology, or terminally ill populations desirable
- Current CPR certification
- Bilingual (Spanish) skills preferred
