Essential Functions:
- Provide personal aspects of care to patients in order to increase level of comfort and maintain personal hygiene.
- Assist in maintaining a safe, healthy environment for patients and families.
- Meet with the Interdisciplinary Team, when appropriate, to assists with review and amendment of the patient’s care plan.
- Practice safely and competently within the job.
- Complete duties according to the Hospice Aide assignment sheet.
- Report significant changes in patient’s condition to registered nurse.
- Maintain accurate and legible records of duties performed per Hospice documentation policies.
- Provides assistance with mobility, transferring, walking, grooming, bathing, dressing or undressing, eating, toileting, and or/housekeeping needs that are necessary to prevent or postpone institutionalization.
- Responsible for picking up and distributing outpatient medical supplies to patients weekly.
- PRN employees are required to work two weekend shifts (Saturday shifts) per month to remain in an active status.
Qualifications
Education:
- High School Diploma or GED preferred.
Education:
- Completion of approved nurse aide training program and successful completion of competency evaluation.
- Current CPR certification
Experience:
-
At minimum, one year experience as a Nurse’s aide.
Skills:
- Sensitivity and openness to the physical needs/care of the terminally ill.
- Maturity and ability to deal effectively with demands of the job.
- Must have the ability to read, write, and carry out directions promptly and accurately.
- Good physical and emotional health.
Other Requirements:
- Must have own transportation and provide proof of liability insurance which meets state requirements.
- Must be able to travel to and from all patient homes without assistance.
- Must be able to perform all aspects of the position unassisted.
- Must have good communication skills and techniques with facilities, patients, families, physicians, and office staff.
