What are the responsibilities and job description for the Palliative Care LPN position at Hospice Foundation Of Greater Baton Rouge?
JOB FUNCTIONS:
- Maintain records, reports and forms that are accurate, timely, and contain quality content per documentation policies.
- Make weekly and biweekly phone calls and home visits as needed and document all calls/visits in EMR.
- Participate in IDT meetings.
- Interact with Palliative Care Inter-Disciplinary Team (IDT) to collaborate on plan of care and needs of the patient and patients family.
- Assist team members with monitoring the program and meeting all measurable goals.
- Ensure quality and efficiency of patient palliative care services.
- Provide high-quality nursing care to patients, and work with families, relatives, and/or other primary care persons to enable them to become dependable caregivers.
- Perform any additional duties requested by the Palliative Care Coordinator and/or CEO.
- Educate the community on the Palliative Care Program as needed.
Education:
- Diploma in Practical Nursing
Certification:
- Current Louisiana license, from an accredited school of Practical Nursing.
- Current CPR certification.
Work Experience:
- At least two years of experience as a staff nurse in either an acute care setting (general medical-surgical, oncology, or geriatric) or home care setting.
Special Skills:
- Ability to assess and respond to needs of patients and families in the home.
- Ability to work independently as well as on an interdisciplinary team.
- Supportive and sensitive to needs of patients and families, as well as other staff and volunteers.
- Able to cope with the emotional stress of patient, family and self that ensues when rendering nursing care to dying patients.
- Humanistic attitude toward patient/family care and tolerant of individual lifestyles.
Other Requirements:
- Good physical assessment skills.
- Good communication skills and techniques with facilities, patients, families, physicians, and office staff.
- Must be in good physical condition.
- Must have own transportation and provide proof of liability insurance which meets state requirements.
- Must be able to travel to and from all patient homes unassisted.
