JOB FUNCTIONS:

- Uses the nursing process to establish a nursing care plan in accordance with established policies and standards of care.
- Provides high quality care to patients, works with families, relatives and/or other primary care persons to enable them to become dependable caregivers.
- Maintains records, reports and forms that are accurate, timely (24 hrs for admits/deaths visits), and contain quality content per documentation policies.
- Anticipates patient/family needs and utilizes the interdisciplinary team and community resources appropriately.
- Anticipates and initiates nursing interventions to prevent complications and alleviate symptoms of physical and psychological discomfort so an optimum quality of life may be maintained.
- Anticipates and meets emergency or crisis situations both physical and emotional.
- Teaches the patient and family re: the plan of care, its goals, and the implications of the diagnosis and prognosis.
- Interacts with all members of the interdisciplinary team and provides nursing aspects to help plan the total needs of the patient/family.
Education:
- Preferably a Bachelor of Science in nursing preparation.
Certification:
- Current Louisiana license.
- Current CPR certification.
Work Experience:
- At least two years of experience as a staff nurse in either an acute care setting (general medical-surgical, oncological, or geriatric) or home care setting.
Special Skills:
- Ability to assess and respond to needs of patients and families in the home
- Ability to work independently as well as on an interdisciplinary team.
- Supportive and sensitive to needs of patients and families, as well as other staff and volunteers.
- Able to cope with the emotional stress of patient, family and self that ensures when rendering nursing care to dying patients.
- Humanistic attitude toward patient/family care.
- Tolerant of individual lifestyles.
Other Requirements:
- Good physical assessment skills and computer skills.
- Good communication skills and techniques with facilities, patients, families, physicians, and office staff.
- Installation of company communication application on cellular device.
- Must be in good physical condition.
