About Our Hospice Social Work Role
Hospice Home Care is seeking a compassionate and skilled Hospice Social Worker to join our interdisciplinary team. As a key member of our organization, you will play a vital role in providing medical social work services to patients and their families.
Responsibilities and Qualifications
- Assess the psychosocial status of patients and families, communicating findings to the interdisciplinary team.
- Provide social evaluations, including assessments of family dynamics, caregiver abilities, and communication patterns.
- Counsel patients and families as needed to address stress and coping difficulties.
- Develop and implement interventions based on evaluation findings.
- Maintain clinical records and educate patients and families on advanced directives.
- Provide information and referral services to patients and families regarding practical and environmental needs.
- Serve as a liaison between patients, families, and community agencies.
Requirements and Experience
- A graduate degree in social work (MSW) from a Council on Social Work Education-accredited program.
- Minimum one year of experience in healthcare, with hospice experience preferred.
- Current licensure to practice in the state of operation.
