JOB DESCRIPTION
ADMISSIONS REGISTERED NURSE

I. QUALIFICATIONS:
A. Graduate of an approved school of professional nursing.
B. Two (2) years of general medical-surgical nursing preferred.
C. Minimum of one (1) year of nursing experience in home health or community health preferred.
D. Recipient of a current license by the State Board of Nurse Examiners.
II. SCOPE:
The Admissions Nurse is a registered nurse who represents Hospice in the Pines services to patients and families in a manner that results in the admission of appropriate patients.
The Admissions Nurse is a field position that interacts with patients, families, and referral sources as needed.
The Admissions Nurse is a member of the admissions team participating in the growth of the hospice program.
The Admissions Nurse works closely with patient care reporting all new admits to the interdisciplinary team.
The Admissions Nurse may also be included in other patient care functions.
III. JOB KNOWLEDGE:
A. Ability to be self directed and function independently. Demonstrate problem solving skills, critical thinking and organizational multi-tasking.
B. Ability to understand the physical, psychosocial and spiritual concerns/problems of terminally ill patients and families.
C. Ability to assess, evaluate and perform the necessary clinical skills involved in providing skilled nursing care to patients in the home or inpatient setting.
D. Demonstrate problem solving skills.
E. Demonstrate understanding of disease progression, dying process, and expected changes.

F. Ability to work consistently under pressure and adapt to frequent changes in schedule. To be objective, to keep feelings, prejudices and personal problems from having a negative impact on work performance.
IV. FUNCTIONS:
A. Explain Hospice services to referral source and/or family.
B. Maintains the financial parameters in regards to medication, supplies, and equipment coverage; seeks approval as needed.
C. Coordinates admission with appropriate staff to achieve an admission goal rate set forth by the agency.
D. Contacts family to set time for evaluation/admission visit. Contacts patient's attending physician and medical director for initial admission order.
E. If non-admit; refer to a more appropriate agency, if applicable.
F. Conduct thorough evaluation and assessment in accordance with clinical parameters during initial visit of patient/family. Has full understanding of criteria and co-morbidities affecting admission.
G. Has the ability to present clinical information to the physicians in a concise and clear manner, demonstrating with the presentation the reason the patient meets criteria for admission.
H. Able to make home visits when necessary.
I. Completes all necessary paper-work and EMR documentation for each patient chart in a timely manner to ensure success of the IDT.
J. Documents visits and phone calls on an ongoing basis for the clinical record, while maintaining accurate patient admission files.
K. Complete continuing education per licensure requirements and submit proof to human resources.
L. Rotates on-call coverage with other staff nurses as necessary.

M. Participates in team meetings and agency events.
N. Completes all necessary paperwork for nursing facility/hospital admission.
O. Determines whether plan of treatment meets palliative guidelines.
P. Participates in the quality assurance performance improvement program.
Q. Builds and maintains effective relationships inside and outside the agency.
R. Completes activity sheets on a daily basis.
S. Maintains adequate inventory of medical supplies and necessary car stock.
T. Informs patients and families of opportunities to help Hospice program by designating Hospice as recipient of memorial donations, donating goods to be distributed to needy families, and volunteering their time and talent.
U. All other duties as assigned.
