The Clinical Supervisor/Nursing Supervisor is responsible for ensuring that patient care is coordinated and managed appropriately. The Clinical Supervisor/Nursing Supervisor is responsible for ensuring that care and services are delivered appropriately and for the supervision of clinical personnel.
Essential Job Functions/Responsibilities
1. PATIENTS, REFFERRALS & COMMUNITY PARTNERS:
a. Receives case referrals. Reviews available patient information related to case, patient’s medical diagnosis, prognosis, medications, procedures and clinical course, including performing home visits, to determine hospice/home care needs. Assigns appropriate hospice personnel to case as needed. Conferences with Medical Director regarding any questions about an individual’s eligibility for services.
b. Reviews and evaluates each case through a variety of means such as home visits, conferences, record review and the services provided by clinicians; discusses and verifies impressions, instructs and guides clinicians to promote more effective performance and delivery of quality home care services; and is available at all times during operating hours to assist clinicians as appropriate.
c. Initiates communication with attending physicians, other hospice team members, and agencies as needed to coordinate optimal care and use of resources for the patient/family.
d. Obtains data on physical, psychological, social and spiritual factors that may influence patient/family health status and communicate that data in team meetings to formulate an Interdisciplinary Plan of Care.
e. Maintains up to date patient records so that problems, plans, actions and goals are accurately and clearly stated, and changes are reflected as they occur
f. Coordinates physical care of the patient and instructs primary caregivers, volunteers and home health aides in providing direct care as appropriate.
g. Conducts interdisciplinary team meetings to facilitate discussion of patient/family status in order to evaluate and revise the interdisciplinary team plan of care.
h. Provides appropriate support at time of death and period of bereavement as needed.
i. Document accurately and appropriately on the laptop, uploading by midnight.
j. When appropriate, enter insurance authorization requests within 24hrs of initial evaluation and continue to enter additional requests at the end of each authorization period.
k. Ensure the clients have prescribed medications and/or treatment supplies, updating file as changes occur.
l. Attends case conference meetings with hospice personnel to facilitate coordination of care and discussion of interdisciplinary group involvement.
m. Works in tandem with Director of Patient Care Services to assure consistency amongst staff with any changes or updates in ALF’s, SNF’s or MD office requirements.
n. Participates in public relation and community activities that promote the organization’s role as an effective member of the hospice and health care delivery system.
2. SUPERVISOR DUTIES
a. Acts as a resource for staff.
b. Assists clinicians in establishing immediate and long-term therapeutic goals, in setting priorities, and in developing plan of care.ssure productivity goals of clinical staff are met.
c. Assists in the screening and interviewing process of new employees and makes recommendations for employment of individuals.
d. Assists in the orientation, training and development and mentoring of new employees and current employees. This includes, but not limited to recruitment, orientation process, precepting, establishing patient assignment, setting up supplies, scheduling orientation calendar with other disciplines (MSW, Chaplain, Marketing, Volunteer Coordinator, etc.), providing employees with continuous training, in-services and education, performing employee reviews, exits, counseling, and meetings.
e. Develops staff to meet quality, safety and competency requirements by assisting with training and professional development of new and current staff, annual joint visits, and setting up vacation coverage.
f. Assists the Director of Patient Care Services and other supervisory hospice personnel in the planning, implementation and evaluation of in-service and continuing education programs. Assists in the formulating, revising, implementing, and evaluating organization policies, procedures, goals and objectives, both short and long range.
g. Promotes customer service orientation to all hospice personnel.
h. Shares in providing 24 hour on-call support 7 days a week, as needed. With the ability to assist with making on-call visits when needed.
i. Works in tandem with the after-hours triage service to assist with coverage of emergent visits and referral assignment.
i. Responsible for assisting in the overseeing of the Clinicians:
i. Acts as the first line for resolution of concerns and issues for the staff.
ii. Notifies Director of Patient Care Services of patient/staff concerns.
iii. Assures productivity goals of clinical staff are met.
iv. Oversees Assignment of patients to primary nurse, adjusts schedules as needed.
v. Regular communication with support staff to assess current needs, determine level of accountability, assign patients and provide supportive discussion related to areas of responsibility and job duties.
j. Maintain a high level of competency, growth and development, and strict adherence to:

3. QUALITY ASSURANCE PERFORMANCE IMPROVEMENT DUTIES:
a. Conducts quarterly record reviews and communicates findings and recommendations to Clinical Director/Director of Patient Care Services and hospice personnel.
b. Participates actively in quality assessment performance improvement program as well as other agency and community programs.
c. Communication with the Director of Patient Care Services regarding continuous quality improvement, problem solving, and other issues related to the overall effectiveness of the hospice program.
d. Assesses health needs of community and assists in the development of plan(s)/ program(s) to meet needs.
4. Complies with accepted professional standards and principles. Maintains confidentiality at all times.
5. Performs other duties and activities as delegated by the Clinical Director/Director of Patient Care Services.

Position Qualifications
1. Registered nurse with current licensure to practice professional nursing in the state.
2. Graduate of National League of Nursing accredited school of nursing with a BSN degree is preferred or a minimum of one (1) year of experience as a professional nurse within the last three (3) years.
3. Previous experience in hospice/home care setting and with two years management or supervisory experience. Proven ability to work within an interdisciplinary setting.
4. Complies with accepted professional standards and practice.
5. Has excellent observation, good nursing judgment and communication skills. Understands hospice philosophy and issues of death/dying.
6. Understands principles of pain/symptom management.
7. Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
8. Complies and maintains current CPR certification.
9. Demonstrates excellent observation, verbal and written communication skills.
