As a Patient Care Advocate, you will support and partner with current Liaisons to identify new referral sources and maintain ongoing relationships to increase program census and preference for hospice services. Patient Care Advocates use their hospice knowledge, empathetic communication skills, and marketing abilities to initiate one-on-one dialog with referral sources, families, and prospective patients to help explain and educate on hospice services. Our Patient Care Advocates are hospice experts and serve as a resource to our Liaisons and referral sources to ensure extraordinary patient and customer service.

Responsibilities
- Supporting Our Team: Collaborate with our Liaisons to gather crucial patient clinical information, provide admissions support, and actively participate in community events. Your contribution directly impacts our patients' experience.
- Building Brand Awareness: Join our Liaisons in brand awareness campaigns. Together, we aim to make our program the preferred hospice provider in the service area, fulfilling our mission and achieving agency growth and census management goals.
- Educating and Connecting: Conduct initial meetings with patients and their families, introducing them to the hospice service and our compassionate philosophy. Obtain consent for their care and make a profound difference in their lives.
- Reporting and CRM Documentation: Ensure compliance with regulatory standards through meticulous record-keeping. Your prompt and accurate submission of marketing reports is crucial for our growth.
- Key Account Management: Identify key accounts and help shape our strategy by communicating strengths and weaknesses to our Liaisons and Regional Sales Director. Your insights are valuable in our growth plan.
Qualifications
- Bachelor’s degree in related field preferred. Additional experience may be substituted for educational requirement.
- Minimum 1 year of experience working directly in hospice required: 2-3 years' experience preferred.
- Prior customer service or sales experience strongly preferred.
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
$65,000 - $75,000 per year (Average Pay Range). The pay range listed represents a general guideline for the role and is not a guarantee of the final offer. Compensation will be determined based on the selected candidate’s relevant experience and the specific responsibilities of the position. Final compensation rate will be discussed and confirmed at the conclusion of the interview process.
Salary : $65,000 - $75,000
