Hospice of Davidson County is seeking a compassionate, dedicated Nurse Liaison to educate, engage, and promote our hospice services within the medical community. This role plays a critical part in building and maintaining relationships with hospitals, physician groups, and other healthcare facilities to increase awareness and referrals. We are looking for a Registered Nurse (RN) with experience in outreach, relationship management, and driving engagement, who is passionate about making a difference in the lives of patients and families.
Primary Function :
The Nurse Liaison is responsible for educating and promoting hospice services to the medical community, facilitating a smooth referral and admission process to Hospice from area facilities, physician groups and hospitals, and ensuring continuity of the hospice plan of care for HODC patients admitted to area hospitals.
Training:
Orientation to Hospice, Home Health, Inpatient and Palliative Care services and any other training that may be necessary to complete the assigned duties.
Supervision : Director of Patient Access; Chief Clinical Officer
Qualifications :
- Associate degree or nursing school diploma required. Bachelor of Science in Nursing preferred. Certification in Hospice and Palliative Care preferred.
- Licensed as a registered nurse in North Carolina.
- Two years current nursing experience required.
- Maintain current unrestricted North Carolina driver’s license and maintain minimum automobile liability insurance for the duration of employment.
- Ability to work as a hospice team member and function efficiently in a flexible work environment.
- Must be physically capable of providing patient care in various institutional settings to include supporting, moving, and lifting adult patients, and lifting equipment and supplies.
- Must be certified to perform CPR (Cardiopulmonary Resuscitation.)
- Must meet immunization requirements of North Carolina and Davidson County.
- Experience in marketing/sales preferred.
Job Duties :
- Understand and commit to the Hospice philosophy of care, including the importance of treating the patient as a whole being, physically, psychosocially and spiritually.
- Provide education to the medical community about hospice services.
- Enhance HODC’s relationship with key referral sources within the medical community by providing education and consistent contact.
- Act as a liaison between HODC and area facilities in order to enhance physician satisfaction and ensure optimal patient care.
- Act as the contact person for facility case managers/discharge planners for referrals to facilitate a prompt and smooth referral and admission into HODC services.
- Meet with referred patients and families in area facilities to explain hospice services and complete pre-assessments as needed.
- May complete initial assessments in any setting and work collaboratively with the interdisciplinary team to establish the plan of care for the patient and family.
- Contact the patient’s attending physician and Hospice Medical Director as needed regarding initial certifications and physician’s orders for the plan of care.
- Teach and counsel the patient/family about the disease process, medication management and provision of care for the patient.
- Complete daily nursing visits (Monday through Friday) on HODC patients while under general inpatient care in area hospitals to maintain case management responsibilities, insure optimal symptom management, update the comprehensive assessment as needed and maintain continuity of care. During periods of high inpatient volume, coordinate with the DONS or designee to arrange for staff nurses to assist with completion of visits.
- Communicate daily with the hospital nursing staff, case management staff, physicians, family and HODC interdisciplinary team to facilitate appropriate discharge plans from inpatient setting.
- Maintain accurate records of Hospice patient/family care services per Hospice of Davidson County policy.
- Participate in Hospice staff meetings, interdisciplinary team conferences (as needed), staff development events and educational programs.
- Relate professionally and courteously with patients, families, members of the Hospice of Davidson County staff and the medical community.
- Participate in the Quality Assessment/Performance Improvement process and the ACHC accreditation process.
- Comply with all HODC policies and procedures, including attendance and confidentiality standards.
- Other such responsibilities as assigned by the Director of Patient Access and/or Chief Clinical Officer.
- Participates in Hospice sponsored in-service trainings.
- Participates in QAPI program.
- Participates on Hospice committees as needed.
- Participates in representing HODC at community events as needed.
- Performs job duties in accordance with all Hospice policies, procedures, professional standards, and annual competencies
- Abides by the Hospice Professional Code of Conduct.

Status and Hours of Work :
This position is classified as exempt. Usual working hours are 8 am to 5 pm, Monday through Friday, or as needed to perform duties. This position is not eligible for overtime pay.
