Job Summary: PRN Registered Nurse (RN) Inpatient Hospice
Responsible for providing nursing services in the inpatient setting to terminally ill patients and supportive services to family members in compliance with all applicable federal, state & local health and safety codes including LSC(R), laws and regulations, and all applicable policies and procedures of Hospice of East Texas.
Job Qualifications:
- Current Texas RN license
- Documentation of at least two years or equivalent of nursing experience in an acute care setting preferred
- Current CPR Certification as required by policy
- Hospice experience preferred
- Valid Drivers License or State Identification provided upon hire
- Demonstrate ability to relate effectively with physicians, health care professionals, the community, patients/families, and members of the hospice interdisciplinary team
- Proficient with Microsoft Office software relevant to position; ability to master software applications relevant to position (including but not limited to the electronic medical record)
- Ability to read, write and comprehend English
- Employee must be FIT tested for an N95 mask
Essential Functions & Responsibilities:
- Knowledge of local, state, and federal laws for delivery of hospice services
- Applies specific criteria for admission and re-certification for hospice care to establish appropriate levels of care and patients’ eligibility for hospice services in collaboration with HomePlace IDG.
- Implements the individualized plan of care and recommendations for revisions to the plan as necessary
- Ensures nursing care complies with patient’s plan of care
- Coordinate all patient/family services and prioritizes the needs with members of the IDT/IDG
- Provides initial and ongoing assessment of the impact of the terminal diagnosis on the patient’s physical, functional, psychosocial and environmental needs and activities of daily living.
- Assesses risk for pathological grief
- Assesses cultural and spiritual implications of end of life issues
- Maintains awareness of verbal and non-verbal patterns of communication of patients/families
- Manages discomfort and provides symptom relief utilizing specialized nursing skills related to palliative and end of life care
- Educates the patient and family regarding the disease process, self-care techniques, issues related to end of life care, and the processes for dealing with issues of ethical concern
- Initiates appropriate preventive and rehabilitative nursing procedures
- Determines the scope and frequency of services needed based on acuity and patient/family needs
- Demonstrates knowledge of medication administration, interactions and side-effects
- Demonstrates knowledge and competence in pain and symptom control concepts, interventions
- Ensures the case management approach for referral to other services as needed
- Prepares clinical and progress notes that demonstrate progress toward established goals according to policy and procedure guidelines
- Communicates information using current processes and technology available
- Ensures Medical Director, attending physician, and other personnel providing care are informed of changes in the patients’ condition as appropriate
- Supervises LVNs, CNAs, and paraprofessionals providing services to the patient according to regulatory guidelines
- Assigns, supervises, and evaluates CNA work performance and takes corrective action when necessary
- Provides specialized hospice training to other staff, family members and informal caregivers to ensure adequate patient care
- Ensures on-going evaluation of the patient/family response to care
- Assesses the ability of the caregiver to meet the patient’s immediate needs upon admission and throughout care provision
- Evaluates own needs for support and identifies systems to meet the needs
- Assumes responsibility on each shift for reordering patient medications and maintaining inventory of supplies
- Participates in the QA/PI process
- Monitors staff and patient safety
