What are the responsibilities and job description for the QAPI NURSE-LVN position at HOSPICE OF EL PASO, INC?
PURPOSE OF POSITION
To serve as a member of the QAPI team to assist in the implementation and evaluation of the organization’s QAPI Program and other related programs. Provide appropriate and adequate vocational nursing care under the supervision of the Director of QAPI/Compliance, as prescribed by the physician in compliance with applicable LVN licensure and agency policies and procedures.
ESSENTIAL DUTIES AND RESPONSIBILITIES

DUTIES AND RESPONSIBILITIES
- Skilled Nursing Services
- Provides safe, competent, and efficient nursing care for minor acute injuries and illnesses under the supervision of the Director of QAPI/Compliance and Hospice physician.
- Documents and monitors staff and volunteer compliance with required vaccinations and preventative screening. Reports non-compliance to Director of QAPI/Compliance and employee’s supervisor.
- Safely administered and appropriately documents on immunizations and TB skin tests given to Hospice El Paso employees. Notifies Director of QAPI/Compliance of any abnormal results.
- Completes COVID-19 testing and results reporting as assigned.
- Report notifiable conditions to the appropriate authorities (for example, the local health department).
- Conducts N95 mask fit testing for all patient care staff upon hire, annually, and as needed.
- Conducts urine drug screens on new employees and assigned. Reports abnormal results to Director of QAPI/Compliance and HR.
- Maintains and ensures confidentiality of patient/employee information and medical records.
- Assist clinical team with patient care as needed.
- Administration
- Performs comprehensive, concurrent, and retrospective quality assurance reviews and audits for identification of deficiencies related to direct and indirect quality care.
- Organizes and compiles QAPI data for analysis and trending.
- Report findings to Director of QAPI/Compliance and/or appropriate department supervisor.
- Assists in communicating QAPI goals and objectives to ensure processes, products, and patient care is improved. Provides feedback to employees of measures and indicators of quality care.
- Assists with training staff on the QAPI Program. Provides instruction on QAPI Program standards to improve on identified deficiency trends.
- Demonstrates cost control in all practice activities.
- Assists in organizing QAPI Committee and other related committee meetings.
3 .Engages in other acts that require education and training, commensurate with the nurse’s experience, continuing education, and demonstrated
competency.
- Complies with agency policies and procedures, mission, and values statement.
- Responds to phone calls, emails, and other messages in a timely manner.
- General
- Shall know and conform to the Texas Nursing Practice Act as delineated in Title 3, Subtitle E, Chapter 301 of the Texas Occupations Code, the Board of Nurse Examiner’s standards, rules and regulations as delineated in Title 22, Part 11 of the Texas Administrative Code, as well as all federal, state, or local laws, rules or regulations affecting the nurse’s current area of nursing practice.
- Maintains Licensure as required by Title 3, Subtitle E, Chapter 301 of the Texas Occupations Code and Title 22, Part 11 of the Texas Administrative Code.
- Notifies Hospice immediately of any pending action against the license or pending suspension or any suspension or action taken against the license.
- Has knowledge of and follows Hospice policies and procedures.
- Maintains a professional appearance.
- Displays a consistent positive attitude.
- Adheres to Hospice of El Paso Mission and Value Statement.
- Performs other duties as assigned.
COMMUNICATION
- Establishes and maintains effective communication and cooperative manners with patients, team members, physicians, hospital, nursing home and foster care personnel.
- Establishes and maintains effective communication and a cooperative manner with Hospice administration, volunteer and patient care departments, all members of the team, and all employees of Hospice of El Paso.
- Comply with HIPAA standard by abiding with the minimum necessary Protected Health Information to perform job function.
PROFESSIONAL DEVELOPMENT
- Attends and participates in inservices, education classes and on-the-job-training programs as required to maintain competency levels.
- Participates in initial competencies upon hire and annually thereafter.
- Attends workshops and seminars as approved.
- Participates in orientation of employees, volunteers, and students as assigned.
- Involved in community/civic health matters/projects.
- Attends ethics and compliance training and maintains compliance with the CCP.
WORKING CONDITIONS
Works in an office setting, as well as throughout the nursing service areas.
- Subject to frequent interruptions.
- Involved with physicians, patients, family members, personnel, visitors, government agencies/personnel, etc., under all conditions/circumstances.
- Subject to hostile and emotionally upset patients, family members, etc.
- A willingness to work beyond normal working hours, and in other positions temporarily, when necessary.
- Subject to falls, burns from equipment, infectious disease, odors, etc., throughout the workday.
- Exposure Level – Category I. Direct contact with blood and other bodily fluids to which standard precautions apply. Personal protective equipment is made available and must be worn.
QUALIFICATIONS
- Education and Experience
- Current licensure as a Licensed Vocational Nurse in the State of Texas.
- Two years clinical experience. Background in oncology, care of terminal patients or community nursing preferred.
- Working knowledge of nursing procedures.

- Demonstrate effective communication skills.
- Ability to work independently and with flexibility.
- A personal vehicle available for transportation during work hours.
- Valid driver’s license and auto insurance as required by the state of Texas.
- Has access to a personal phone at home.
PHYSICAL REQUIREMENTS
- Sits, bends, lifts, and walks intermittently during working hours. Can sit for prolonged periods.
- Can reposition up to 50 pounds of weight.
- Possesses sight/hearing senses or uses prosthetics that enable these senses to function adequately, so that the requirements of this position can be fully met.
