About the Role: We are seeking a skilled Registered Nurse Case Manager to join our team. As a RN Case Manager, you will provide compassionate nursing care to patients in their homes or living facilities.
Responsibilities:
- Nursing Assessments: Conduct thorough assessments of patient needs and update plans of care accordingly.
- Patient Care: Provide direct nursing care to maintain patients' highest level of comfort and quality of life, regularly assessing for nursing care needs.
- Interventions: Initiate appropriate interventions and monitor outcomes to ensure optimal patient care.
- Education and Coordination: Educate patients and family members on necessary medical equipment, supplies, and services, and coordinate hospice nursing care when outside agency involvement is required.
- Schedule and Availability: Work Monday through Friday from 8:00 a.m. until 5:00 p.m., with occasional on-call coverage as needed/scheduled.
Qualifications:
- Licensure and Education: Must be a graduate of an accredited school of nursing and hold current Registered Nurse (RN) licensure in the State of Michigan.
- Clinical Experience: Two years of recent clinical experience is preferred, but recent nursing school graduates are also encouraged to apply.
- Clinical Judgment: Possess sound clinical judgment and knowledge of pain symptom management with end-of-life care.
